Waiver / Pass Forms
Every rider at MAWI Cable Park must have a waiver on file for each calendar year. To get your waiver, click the button below. The form will open in a .pdf format, you can fill it out, get it signed, and you are good to go!
If you are under 18 years of age, you must have the form signed by your parent or guardian.
If the form is signed other than in front of a MAWI employee, it must be notarized. Your parents can get see a Notary Public at any bank or Tag Agency. A Notary Public certifies that the signature is authentic.

The "Season Passes" are gone! Now, if you purchase in January, for example, you have pass privileges thru next January! If you buy in July, you are good thru next July! 12 full months of riding the cable at MAWI 151 Wake Park!
And you can do it all via fax and email if you desire!
